Due to regulatory requirements, the application process is complex. We have tried to streamline the process as much as possible and we will work closely with you to ensure that all necessary steps are completed as quickly as possible.

Currently there are two versions of our application available:

1. An interactive Microsoft Word Document form or

2. A PDF version that you can print and fill out by hand

The application process is more stringent for Workers’ Comp than for the Vehicle Pool because of State underwriting requirements.  Typically, application needs to be made at least 6 weeks prior to the applicant’s desired bind date to accommodate this process.
